Changes:
- Add a hotfix for lack of udev synchronization on
  --ignorelockingfailure (and --sysinit).
- Use --sysinit in lvm2 hook.
- Remove 'vgscan' from lvm2 hook.
- Do not add dmeventd to the initramfs.
