If my memory serves me correctly, you said that your card is read in
Windows.

What I would suggest you is to format it in Windows, but from the
command prompt (start menu, type "cmd" or just browse accessories and
start "command prompt"):

In the terminal/command prompt type:

diskpart.exe
list disk
select disk X
detail disk
clean
create partition primary
active
format fs=fat32 quick
assign
exit
